How to Complete the F.O.I. Application Form

(Access/Correction Request)

Please ensure the following has been completed prior to submitting your request…

• Fill in your date of birth.
• Fill in your full name, address and telephone number.
• Clearly describe what information you are requesting.
• List all particulars ( i.e. date, time, occurrence number, officer’s name & badge number, name of other individuals involved, etc).
• Sign and date the form.
• Pay the $5.00 application fee for each request. A separate form and fee must be submitted for each request.
• A copy of a drivers license must be included with the FOI application.
